Quizam Pilot Program Agreement

The Quizam Pilot program is for schools and institutions
that would first like to try out Quizam.

Terms:
• I agree to download Quizam and the online quizzes.
• I agree to take the online customer survey.
• I understand Quizam Media Corporation is a public company and that a news release will be posted about our participation.

Quizam Media Corporation will:
• Provide a unique password for registered users.
• This password is for the entire school, including parents.
• Quizam will issue a 6 month licence.
• The 6 month licence entitles your school to download the most current version.
• Schools that purchase Quizam recieve a 30% discount of the retail price of $449.00
• Districts will recieve a special price based on student usage.

Yes our school/institution would like to participate in the Quizam Pilot Program.
